*We do apologize, but due to high demand and to the personalized nature of our process, new patient bookings are delayed. Are you still interested in scheduling an appointment?

*Are you willing to work with one of our Nurse Practitioners and staff, to follow the "Take It Back Method" for the best results and outcomes? Generally speaking you should plan on at least 6 months of treatment.

*Are you willing to see one of our Healthcare Providers every 4 months or as directed by Joseph Debban APRN to monitor your journey?

* Our services are not covered by insurance. By working outside of insurance, we are able to offer high-quality care at lower costs and with greater flexibility. Is this something you would like to participate in?

*Ongoing care at J Debban Wellness requires an active membership at $75/month. This includes quarterly visits, support, and discounted labs/supplements. Are you willing to join as a member?

*To reserve your appointment, a credit card is required. We ask for at least 72 hours notice if you need to cancel or reschedule. Appointments cancelled late or missed without notice will be charged a $100 fee.

*You will receive an email with important information and intake forms. Completing these forms in advance helps us get to know you and provide the best possible care. Are you able to receive email and complete the required forms prior to your appointment?

*1. Wellness Intake with a Registered Nurse. This visit includes lab work, a review of your health history, current medications, and completion of assessment forms.

*2. Take It Back Strategy Session with one of our Nurse Practitioners. This visit includes a review of your lab results a personalized wellness options to decide whether you'd like to move forward with care. This appointment length is 60 minutes.
